Community Partners

Our Community Partners, a partnership with IRIS, is a support for families with children 0-18 that have requested support or referred by someone else. The program provides home-based case management services to connect families to resources to strengthen the family. Families work with their case managers on mutually established goals to move the individuals and family unit towards economic and family stability.

Community Partners (CP)

In partnership with IRIS, we offer Community Partners services as support for families with children ages 17 or younger living in the home. We can also serve families when they are prenatal or if they have an 18 year old still in school. This program, administered by IRIS, funded by the Indiana Children & Family Services, provides assessments and home-based case management services to connect families to resources to strengthen the family. Families work with their case managers on mutually established goals to move the individuals and family unit towards economic and family stability. Through this program, families can access services such as:

  • Housing crisis assistance
  • Financial education and planning
  • Home scheduling assistance
  • Navigation of a medical diagnosis
  • Job Search
  • And so much more!

CP Outcomes

  • 97% of clients increased in their parenting capabilities.
  • 95% of clients showed an increase in Financial Management skills
  • 96% of clients increased in food and nutrition skills
  • 97% of clients demonstrated an increase in family access to health and mental health care
